Home Care Package
The Home Care Package program offered at Hawthorn Grange allows our Supported Residential Service residents a choice in accessing a home care package to support care and lifestyle goals and assist with keeping the care affordable for you.
Hawthorn Grange will be considered your private home, thus allowing you to still access your Home Care Package to assist in the funding of your personal care. Home Care Package services are available to residents living within Hawthorn Grange with all services provided by the quality care team who you know and trust.
What is a home care package?
A Consumer directed Home Care Package is a coordinated
package of Government funded care.
Packages range from Level 1 (Basic Care needs) through to Level 4 (High Care Needs).
Eligibility for a home care package is based on your care needs and is assessed by the Aged Care Assessment Service (ACAS)
What services can be provided under a home care package?
Allied health – including Physiotherapy,
occupational therapy, speech pathology
How does it work within the Supported Residential Service?
Following an assessement and eligibility criteria being established, you will receive an offer of funding from the Government. A
Hawthorn Grange representative will meet with you and your family to discuss our program and what care and services you would like your home care package to provide.
A home care package service agreement is provided, a care plan is prepared which will identify the home care package services and provide you with an individualised budget. This care plan is incorporated into your SRS care plan with identification of what services are being provided from which program and how this is funded.
