At Hawthorn Grange in Hawthorn, we offer permanent supported accommodation for people who are unable to live independently and require daily assistance. This includes older people, individuals living with disability, or those recovering from illness or injury.

 

Our home is located in Melbourne’s inner south-eastern suburbs, and provides 24-hour care in a comfortable and supportive residential setting.

 

We offer a full range of services to meet both your physical and emotional wellbeing, including:

We also understand the importance of staying connected. Our community-focused approach encourages friendships, family visits, and active engagement in daily life.

 

Home Care Packages can be used to complement the care provided at Iris Manor, adding additional services such as specialised nursing, one-on-one support, or allied

At Hawthorn Grange, each resident has a private single room with an ensuite, telephone, and a 24-hour emergency call button for peace of mind. Our caring team, including Division 1 Nurses, is available around the clock to provide professional support when you need it. All meals are freshly prepared onsite with a focus on nutrition and variety, and daily cleaning and laundry are included to make life easier.

 

You’ll feel right at home with comfortable dining and lounge areas, quiet spaces for family visits, and beautifully landscaped gardens with a sheltered courtyard. We offer a discounted pharmacy program, onsite hairdressing and podiatry services, and a full calendar of in-house activities and local outings to help you stay connected, active and engaged.

If you're receiving a Home Care Package, Iris Aged Care can help you make the most of it.

Hawthorn Grange is proudly owned and operated by Iris Aged Care, a family-run aged care provider supporting older people across Hawthorn, Melbourne, and the surrounding south-eastern suburbs.

Iris Aged Care offers in-home care and support through government-funded Home Care Packages, as well as 24/7 care at Hawthorn Grange. Their approach is built on trust, respect, and a deep commitment to personalised care.

Frequently asked questions

  • A Supported Residential Service, offers accommodation and personal care for people who need help with daily activities but don't qualify or who might not want to use government-subsidised aged care. Unlike aged residential aged care homes, Supported Residential Service facilities do not receive government funding and can often provide more flexible or personalised care.

    A Supported Residential Service does not require payment of a Refundable Accommodation Deposit (RAD) or Daily Accommodation Payment (DAP).

  • Hawthorn Grange provides around-the-clock care tailored to meet our resident’s unique needs.

     

    An Supported Residential Service (SRS) provides assistance with daily living activities, such as personal care (bathing, dressing), meal preparation, medication management and support with mobility.

  • As Supported Residential Service facilities do not receive government subsidies, the cost of accommodation and services can vary depending on the level of care required. Weekly fees cover accommodation, personal care, meals, laundry, cleaning and access to services. Additional fees for extra services are applicable where required.

    A Supported Residential Service does not require payment of a Refundable Accommodation Deposit (RAD) or Daily Accommodation Payment (DAP).

  • Yes, Home Care Packages can be used to complement the care provided by Hawthorn Grange. This allows our clients to access services, such as specialised nursing care, personal support, or allied health services, using available Home Care Package funds.

  • Home Care Packages can fund a variety of services at Hawthorn Grange, including one-on-one personal care, specialised nursing (e.g., wound care or medication management), allied health services (physiotherapy, podiatry), meal preparation, and transport for medical appointments. These services are tailored to specific needs through an individualised care plan.

  • No, Home Care Packages are not designed to cover accommodation costs in a Supported Residential Service. The package funding is intended for care services and support. The cost of staying in a Supported Residential Service, such as accommodation, personal care, meals, laundry, cleaning, is usually privately funded by the resident or their family.

  • Yes, even if you’re receiving 24-hour care at Hawthorn Grange, your Home Care Package can be used to fund, specific services that are included in the standard Supported Residential Service care.  

    An example of this is that the HCP could be used to fund a portion of the personal care or nursing care provided daily. Your Home Care Package could also be used to fund allied health services, specialised nursing care, or assistance with personal tasks that require extra attention whilst you are living at one of our care facilities.

  • If you already have a Home Care Package, there’s no need for a new assessment. We will review your current needs and work with you to create a suitable care plan. If your needs change over time, we’ll guide you through the reassessment process via the Aged Care Assessment Team (ACAT).
